What this service is
DementiaMonitor offers a self-reported checklist of memory concerns and saves your results so you can compare them over time. It is an educational and wellness tool. DementiaMonitor.com is operated by Lost Property Finder LLC, 2045 W Grand Ave Ste B PMB 71442, Chicago, IL 60612.
What this service is not
This is not a medical service. The check is not a medical test, not a diagnostic tool, and not a substitute for a doctor. Your score cannot tell you whether you have dementia or any other condition. Only a qualified clinician can evaluate memory or cognitive concerns. If you are worried about your memory, talk with your doctor. If you have an urgent medical concern, call your doctor or emergency services.
Using the service
The check is free. You agree to give a real email address so we can save your score for you. You may request deletion of your data at any time (see the privacy policy).
No warranties
The service is provided as-is. We work to keep it accurate and available, but we make no guarantees, and we are not liable for decisions made based on a score from a self-reported checklist.
Attribution
The questionnaire is adapted from Maffoni, Pierobon & Fundarò, Frontiers in Psychology (2022), used under the CC BY 4.0 license, with several questions rewritten in our own words.
